A week of unusual fatigue, a butterfly rash across her cheeks, and joint pain that made grading papers feel impossible. She had been taking her Plaquenil every morning without fail. Her last rheumatology visit had gone well. And yet, here she was — flaring.

Her first thought was that the medication had stopped working. Her second thought, absorbed from an online forum she'd visited at 2 a.m., was that it had never been working at all. Neither conclusion, as her rheumatologist would explain at her urgent appointment, was accurate.

Sarah's experience is far from unusual. Breakthrough flares — episodes of increased disease activity that occur while a patient is adherent to hydroxychloroquine therapy — are among the most confusing and distressing events in the course of autoimmune disease management. Understanding what they mean, and what they do not mean, is essential for navigating them without abandoning a treatment regimen that may still be providing meaningful benefit.

Hydroxychloroquine Is a Disease Modifier, Not a Disease Cure

The first and most important context for understanding flares during Plaquenil therapy is a clear-eyed appreciation of what the drug does and does not do. Hydroxychloroquine is classified as a disease-modifying antirheumatic drug, or DMARD. Its mechanism involves modulating immune system activity — reducing the inflammatory signaling that drives autoimmune damage — rather than eliminating the underlying autoimmune condition.

This distinction matters enormously. A patient on hydroxychloroquine who flares has not experienced a drug failure in the same sense that a patient on a blood pressure medication whose blood pressure spikes has. The autoimmune process is complex, multifactorial, and inherently variable. Disease-modifying therapy reduces the frequency and severity of flares and slows cumulative organ damage; it does not guarantee a flare-free existence.

Clinical studies of lupus patients on hydroxychloroquine, for example, consistently show reductions in flare frequency compared to untreated patients — but not elimination of flares entirely. The goal of therapy is disease control, not disease erasure.

Common Triggers That Can Produce Flares Despite Medication Adherence

When a patient on hydroxychloroquine experiences increased disease activity, the clinical investigation begins not with questioning the medication but with identifying what may have provoked the episode. Several well-documented triggers can overwhelm even effective immunomodulatory therapy.

Infections are among the most common precipitants of autoimmune flares. A respiratory virus, urinary tract infection, or even a course of antibiotics disrupting the gut microbiome can shift immune system balance in ways that activate disease pathways. Patients often notice flares in the weeks following an illness without connecting the two events.

Psychological and physiological stress activates the hypothalamic-pituitary-adrenal axis and produces systemic inflammatory responses that can trigger or amplify autoimmune activity. Significant life stressors — job loss, bereavement, a difficult semester, a family crisis — frequently precede flares by days to weeks.

Sun exposure is a well-established lupus trigger, capable of provoking cutaneous and systemic flares even in patients on stable therapy. Patients who spend increased time outdoors during summer months without adequate sun protection may attribute a subsequent flare to their medication rather than to ultraviolet exposure.

Hormonal fluctuations — including those associated with the menstrual cycle, perimenopause, or changes in hormonal contraception — can influence disease activity in ways that are not fully preventable through medication alone.

Medication interactions or absorption issues occasionally contribute to reduced hydroxychloroquine efficacy. Antacids containing calcium or magnesium can impair absorption if taken concurrently, and gastrointestinal illnesses that affect absorption may temporarily reduce effective drug levels.

When a Flare Signals Something More Than Routine Fluctuation

Not every flare during hydroxychloroquine therapy is benign in its implications. Some breakthrough episodes do warrant a reassessment of the treatment plan, and distinguishing these from expected disease fluctuations requires clinical judgment.

Patterns that may indicate the need for therapeutic adjustment include: flares that are increasing in frequency rather than decreasing over time; episodes that are more severe than those experienced before initiating hydroxychloroquine; flares accompanied by laboratory evidence of worsening disease activity (rising anti-dsDNA antibodies, declining complement levels, changes in complete blood count); or any flare involving organ systems that were not previously affected.

In these circumstances, the rheumatologist may consider whether the current hydroxychloroquine dose is appropriate given the patient's weight and renal function, whether a combination therapy approach — adding agents such as methotrexate, azathioprine, or a biologic — is warranted, or whether the diagnosis itself merits re-evaluation.

This clinical reassessment process is not evidence that hydroxychloroquine has failed. It is evidence that autoimmune disease is dynamic and that treatment plans appropriately evolve alongside it.

What Patients Should Document and Communicate

One of the most valuable things a patient can do when experiencing a suspected flare is to document it carefully before contacting their care team. A brief written record — noting symptom onset, symptom type, recent stressors, recent illnesses, any changes in diet or supplement use, sun exposure, and current medication schedule — gives the rheumatologist a far richer picture than "I've been feeling worse for a few days."

Patients who keep a simple symptom diary, even an informal one maintained in a phone's notes app, are better equipped to identify patterns over time. A rheumatologist who can see that a patient's flares consistently follow stressful periods or sun exposure can offer targeted guidance that goes beyond medication adjustment.

It is also worth noting what not to do during a flare: stopping hydroxychloroquine without medical guidance is one of the most counterproductive responses, as the drug's long half-life means that any protective effect will persist for some time after discontinuation — but restarting after a gap requires weeks to months to re-establish therapeutic levels. The flare may pass; the treatment interruption creates a longer-term vulnerability.

Reframing the Flare: From Failure to Information

For patients like Sarah — and for the many others who have sat with the disorienting experience of flaring on a medication they were told would help — reframing the flare as clinical information rather than clinical failure is a meaningful shift in perspective.

A flare on hydroxychloroquine is not proof that the medication is not working. It is a signal worth decoding. It may point to a trigger that can be addressed, a dosing question worth raising, or a treatment plan that has reached the limits of what a single agent can accomplish. In each case, the appropriate response is engagement with the care team — not retreat from the treatment itself.
